Duties
- Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and toileting to ensure their comfort and dignity
- Monitor patient observations by tracking vital signs and recognizing changes in health status, promptly reporting findings to healthcare professionals
- Support medication administration following medical instructions and maintaining accurate medical documentation in Electronic Health Record (EHR) systems
- Provide specialized care for clients with dementia, Alzheimer's disease, or other memory-related conditions, including memory care techniques and behavioral management
- Operate medical equipment such as Hoyer lifts for safe patient transfers and catheter care devices as needed
- Assist with meal preparation, feeding, cleaning, laundry, and light housekeeping to maintain a clean and healthy living environment
- Document all care activities accurately while adhering to HIPAA regulations to ensure patient confidentiality and compliance with healthcare standards
Skills
- Proven experience in home health or long-term care environments, including assisted living or nursing homes
- Strong knowledge of medical terminology, patient care procedures, and documentation practices using EMR/EHR systems
- Ability to perform patient observation skills effectively and recognize signs of health deterioration or behavioral changes
- Proficiency in assisting with ADLs, medication administration, and specialized dementia or hospice care techniques
- Familiarity with medical equipment such as Hoyer lifts, vital sign monitors, and catheter care tools
- Excellent communication skills for working with patients with disabilities or developmental challenges and collaborating with healthcare teams
- Valid CPR certification along with knowledge of safety protocols for patient handling and emergency response procedures
