About the Position:

As an Outreach Support Worker, you will provide participant-centered support services across a variety of programs, including housing support, system navigation, crisis intervention, basic needs assistance, and connection to community resources. Your work will be guided by housing first, family first and harm reduction philosophies as well as being participant-focused and strengths based.

Key Responsibilities:

  • Outreach and engagement with individuals experiencing or at risk of homelessness, housing instability, mental health challenges, substance use concerns, or other barriers to stability, as determined by the needs of each individual program.
  • Make timely and appropriate referrals to internal programs, housing agencies, healthcare providers, treatment services, legal supports, income supports, education services, and other community resources.
  • Provide support for at risk youth and their families to promote family reunification.
  • Develop individualized service plans based on participant needs, goals, and barriers, focusing on housing stability, life skills, employment readiness, income supports, and connection to health and social services.
  • Maintain current knowledge of legislation, policies, procedures, and community resources relevant to vulnerable populations and the programs being supported.
  • Build and maintain collaborative working relationships with community agencies, shelters, hospitals, schools, justice services, healthcare providers, landlords, and other support systems to improve participant outcomes.
  • Compile, maintain, and accurately document participant records, case notes, assessments, service statistics, and required reports in accordance with organizational standards and funder requirements.
  • Willing to work a flexible schedule (30 hours per week) as this will vary based on the needs of the program and participants.

Requirements & Qualifications:

  • Degree/Diploma in Social Work, Child and Youth Care Counselling, Addictions Counselling or related human services field. A combination of experience and education may be considered.
  • Minimum one (1) year of experience working with youth and families.
  • Minimum one (1) year experience working in outreach.
  • Detailed knowledge of community resources for participants
  • Experience working with mental health, addictions, homelessness.
  • Strengths in crisis intervention, case management, communication and administrative skills are a must.
  • Clear Criminal Record Check with vulnerable sector and Child Intervention Check dated within the past six months,
  • Clear Driver’s Abstract, valid Class 5 Driver’s License, daily use of reliable vehicle and minimum $2M vehicle insurance to carry passengers.
  • Standard First Aid CPR Level C Certificate.
